If you do not see your insurance company listed, a Super Bill (detailed receipt of services rendered) may be provided. Many (not all) insurers will allow you to submit a Super Bill for reimbursement.

Sliding Scale / Financial Assistance

We do offer a sliding scale for folks who require financial assistance, no questions asked. Please contact us for more information.

To find out if your insurance will cover medical nutrition therapy, call the number on the back of your insurance card and ask the following questions. Be sure to ask for a reference number and save this information someplace secure.

If needed, our group NPI is 1245843440.

  • Does my plan cover outpatient nutrition counseling in a private practice setting? (Service codes we use are 97802 and 97803)

    • If there are no nutrition benefits, you may be responsible for the fee-for-service.

  • If I do have nutrition benefits, does it only cover certain diagnoses? (ex: some plans will only cover Type 1 diabetes. Most insurance companies won’t tell you directly which diagnoses they cover.)

  • Does my policy require a referral, pre-authorization, or pre-determination?

  • How many visits do I qualify for each calendar or plan year?

  • What is my co-payment or co-insurance due at each appointment?

  • Do I have a deductible, and if so, will the cost of my nutrition appointment be applied to my deductible? (In this case, the patient will be responsible for the cost of the session out-of-pocket until the deductible is reached.)

  • Do my benefits cover telehealth for nutrition counseling?
