Insurance Script:
To find out if your insurance will cover medical nutrition therapy, call the number on the back of your insurance card and ask the following questions. Be sure to ask for a reference number and save this information someplace secure.
If needed, our group NPI is 1245843440.
Does my plan cover outpatient nutrition counseling in a private practice setting? (Service codes we use are 97802 and 97803)
If there are no nutrition benefits, you may be responsible for the fee-for-service.
If I do have nutrition benefits, does it only cover certain diagnoses? (ex: some plans will only cover Type 1 diabetes. Most insurance companies won’t tell you directly which diagnoses they cover.)
Does my policy require a referral, pre-authorization, or pre-determination?
How many visits do I qualify for each calendar or plan year?
What is my co-payment or co-insurance due at each appointment?
Do I have a deductible, and if so, will the cost of my nutrition appointment be applied to my deductible? (In this case, the patient will be responsible for the cost of the session out-of-pocket until the deductible is reached.)
Do my benefits cover telehealth for nutrition counseling?
